"We need a Journal of Outstanding Clinical Failures.”

We also need a journal of rival hypotheses: the whole Prempro scandal could have been avoided if there had been sufficient devil's advocacy in the journals during the time all the cheerleading articles were being published.

It's easy in hindsight to say x or y article shouldn't have seen the light of day. But peer reviewers won't be suspicious if they don't have any reason to think the science is 'off'. Lots of people thought the science was plausible, and now lots of people are scrambling to compensate for their credulity....and not always in productive ways.
