Introducing The Map of Medicine

Map of Medicine is a visual representation of evidence-based, practice-informed care pathways. A knowledge resource that supports clinically-led service improvement programmes, the Map has been shown to improve patient outcomes and lower healthcare delivery costs.

The Map works with multidisciplinary clinical stakeholders, including Royal Colleges and Clinical Societies, to create complete care pathways that are continuously maintained to reflect the latest evidence, expert opinion and cognisant of policy information. Used as a starting point by healthcare economies and communities looking to redesign services, the pathways are an ideal approach to care from a clinical perspective. Local clinical groups agree how their particular challenges will be met and adapt the pathway to reflect their resulting service design decisions.
Locally-adapted pathways on the Map are visible across organisations, allowing local innovation to be shared quickly and establishing a benchmark for the clinical practice expected of providers.

More than 50 local communities are reaping the rewards of using the Map. See some case studies to learn how they are benefiting from lower healthcare delivery costs, improved clinical effectiveness and patient safety and an improved patient experience, such as reduced hospital admissions and lowered waiting times.

Map of Medicine pathways are also at the heart of the Intel Health Guide, currently being piloted by NHS Central Lancashire for patients with COPD. Map pathways are used to develop care plans for use on the Intel Health Guide that are based on best practice and evidence.
